    Fill out the form below to attest the fact that a strategic partnership exists between your business and a business participating in the Connecticut Insulation Installer Network (CTIIN) or the Home Energy Solutions (HES) program. You'll need to have a business relationship in place and the contact information for your business partner to complete the form.
  • Purpose of attestation:

    This Attestation confirms that a formal business relationship exists between the undersigned Parties and sets forth each Party’s agreement to certain terms regarding their collaboration in connection with the Energize Connecticut Program. The Attestation does not set forth all the terms of the business relationship between the Parties, and additional written agreements between the Parties are required in order to fulfill the requirements which are adopted herein.

    ATTESTATION:

    This Attestation is made on this day of {agreementDate}, by the following Parties:

    - Party 2: {party2}, with its primary place of business located at {party249}

  • Party 1 hereby attests that Party 1 is a member in good standing of the following Energize Connecticut Program Networks (check all that apply):*
  • Party 2 hereby attests that Party 2 is a member in good standing of the following Energize Connecticut Program Networks (check all that apply):*
  • Party 1 and Party 2 hereby jointly and severally attest as follows:

    A) We have entered into a Business Relationship (defined below) whereby we mutually intend to promote the goals of the Energize Connecticut Program through our collaboration on delivering energy-savings improvements to existing or prospective Customers.

     

    a. “Business Relationship” is defined as a collaborative association between a Party who is a member of one or more Energize Connecticut Program Networks (identified above) and a different Party who is a member of one or more different Energy Connecticut Program Networks (identified above), in which the collaborative association i) is reflected in a written agreement between the two Parties; and ii) fulfills one of the following criteria:

    i. Subcontractor Relationship: one Party (Subcontractor) agrees to perform services for the other Party (General Contractor) as a subcontractor, whereby a Customer may enter into a contract with the General Contractor that will govern the delivery of all aspects of energy-savings improvement services, including but not limited to installation, warranties, insurance, protection of Customer information, and compliance with laws.

    ii. Lead Sharing Relationship: each Party (in such case the Referring Party) agrees to introduce, on a non-exclusive basis, existing or prospective Customers to the other Party (Receiving Party) for purposes of the Receiving Party entering into agreements with such Customers for the delivery of energy-savings improvement services which the Referring Party does not provide in-house or pursuant to a subcontract, provided in all cases that Customer information may only be disclosed with the Customer’s consent and in accordance with all applicable legal and contractual requirements.

     

    B) Our Business Relationship is in effect at the time of this Attestation, and in the event either Party terminates the Business Relationship in the future we each agree to promptly provide notice of the termination to the Energize Connecticut Contractor Support Team by email to EnergizeCT@AbodeEnergy.com.

     

    C) We each consent to the public disclosure of our Business Relationship by the Energize Connecticut program as well as by the other Party, and we each grant permission for the joint display of our business names, logos, Energize Connecticut Partner Network memberships, and a brief description of our Business Relationship in marketing materials, websites (including www.EnergizeCT.com), or other promotional platforms and in communications to existing or prospective Customers.

     

    D) We each commit to using our best efforts to provide Customers with a holistic, integrated, and Customer-focused approach to the energy-savings improvement services that we deliver, including but not limited to:

    a. Offering subcontracted services or lead sharing, as applicable, when requested by Customers or when a comprehensive approach is appropriate or advisable to serve Customers optimally and efficiently;

    b. Emphasizing the value of joint energy-savings improvement services to Customers in marketing and sales communications, including educating both Customers and our own team members regarding the benefits of improved energy efficiency, comfort, and cost savings that can achieved through delivery of integrated energy-savings improvement services.

    c. Communicating and collaborating among each other effectively and in a timely manner to coordinate services and promote a seamless experience for Customers, and particularly to share and review Customer feedback and to address all Customer satisfaction issues or concerns that may arise in an effective and integrated way;

    d. Protecting Customer information in accordance with legal requirements and best business practices, including each Party maintaining and abiding by a written information security policy.
